Why diabetes affects the eyes
High sugar levels damage the tiny vessels in the retina.
Over time, raised blood sugar weakens the small blood vessels of the retina. They can leak fluid, bleed, or close off, and the eye may grow fragile new vessels in response.
When fluid collects at the centre of the retina (the macula), reading and recognising faces become difficult. This is called diabetic macular oedema and is a common cause of vision loss in people with diabetes.
- Often no symptoms in early stages
- Affects both type 1 and type 2 diabetes
- Risk rises with how long you have had diabetes
Stages
How diabetic retinopathy progresses.
Early
Mild to moderate
Small bulges and leaks in retinal vessels. Usually needs monitoring and good sugar control.
Moderate
Severe non-proliferative
More vessels are blocked and the retina is short of oxygen. Closer follow-up is needed.
Advanced
Proliferative
Fragile new vessels grow and can bleed or pull on the retina. Needs prompt treatment.
Macula
Diabetic macular oedema
Swelling at the centre of the retina that blurs reading vision. Can occur at any stage.
Who should be checked
Everyone with diabetes needs regular eye checks.
Most people should have a dilated retina examination at least once a year, or as advised by their doctor.
Anyone with diabetes
Type 1 or type 2, even if your vision seems perfect.
Long-standing diabetes
The longer you have had diabetes, the higher the risk.
High BP or cholesterol
These add to the damage to retinal blood vessels.
Kidney disease
Often goes hand in hand with diabetic eye changes.
Pregnancy with diabetes
Retinopathy can progress faster during pregnancy.
Blurred or patchy vision
Any change in vision should be checked promptly.

FAQs
Common questions.
I have diabetes but my vision is fine. Do I still need a check?
Yes. Diabetic retinopathy usually causes no symptoms until it is advanced. A yearly dilated eye examination can find changes early, when treatment is most effective.
How often should I get my eyes checked?
At least once a year for most people with diabetes. If retinopathy is found, Dr. Somya may advise checks every few months.
Can diabetic retinopathy be cured?
Damage that has already happened may not fully reverse, but treatment can slow or stop progression and often improves swelling. Good control of sugar, blood pressure and cholesterol makes a big difference.
Can I drive after the examination?
Dilating drops blur vision for a few hours, so please don’t drive yourself home. Bring someone with you and sunglasses if you can.
Do I need to stop my diabetes medicines before the visit?
No. Take your medicines and eat as usual. Bring a list of your medicines and your latest sugar reports if you have them.
